I would've thought it'd be in the ILLC's list of historic dissertations
(http://www.illc.uva.nl/Research/Publications/Dissertations/reportlist.php?Series=HDS), 
but it isn't; however if you wrote the ILLC
buro and suggested that it be added, I suspect they'd do so promptly if
they're able.
